20 C.F.R. § 30.415 — What are the requirements for medical reports?

§ 30.415 What are the requirements for medical reports? In general, medical reports from the employee's attending physician should include the following: (a) Dates of examination and treatment; (b) History given by the employee; (c) Physical findings; (d) Results of diagnostic tests; (e) Diagnosis; (f) Course of treatment; (g) A description of any other conditions found due to the claimed occupational illness or covered illness; (h) The treatment given or recommended for the claimed occupational illness or covered illness; and (i) All other material findings.
