25 C.F.R. § 43.15 — Content of consent.
§ 43.15 Content of consent. The consent of a parent or eligible student requested under this part for the release of student records shall be in writing, signed and dated by the person giving the consent. The consent shall include: (a) A specification of the records to be released. (b) The reasons for release. (c) The names of the parties to whom the records will be released.
