Pharmacist Prescribing — General Requirements

TITLE 54 PROFESSIONS, VOCATIONS, AND BUSINESSES CHAPTER 17 PHARMACISTS 54-1734. PHARMACIST PRESCRIBING — GENERAL REQUIREMENTS. (1) A pharmacist may independently prescribe if such pharmacist: (a)  Only prescribes drugs for conditions for which the pharmacist is educationally prepared and for which competence has been achieved and maintained; (b)  Only issues a prescription for a legitimate medical purpose arising from a patient-prescriber relationship; (c)  Obtains adequate information about the patient’s health status to make appropriate decisions based on the applicable standard of care and the best available evidence; (d)  Recognizes the limits of the pharmacist’s own knowledge and experience and consults with and refers to other health care professionals as appropriate; and (e)  Maintains documentation adequate to justify the care provided, including but not limited to the information collected as part of the patient assessment, diagnosis, prescription record, provider notification, and follow-up care plan. (2)  The general requirements provided for in this section do not apply to the prescribing of devices and nonprescription drugs, prescribing under a collaborative pharmacy practice agreement, direct administration of a medication, or prescribing emergency drugs authorized under this chapter.
