Idaho Code § 39-904 — Statement In Report Of Birth.
TITLE 39 HEALTH AND SAFETY CHAPTER 9 PREVENTABLE DISEASES IN INFANTS AND NEWBORNS 39-904. Statement in report of birth. Every physician or midwife shall, in making a report of a birth, state whether the ocular antibiotic prophylaxis described in section 39-903 , Idaho Code, was applied to the eyes of the newborn.
