Idaho Code § 39-245 — Certificate Forms.
TITLE 39 HEALTH AND SAFETY CHAPTER 2 VITAL STATISTICS 39-245. Certificate forms. The form of certificates used under the provisions of this chapter shall be prescribed by the director and shall include as a minimum the items required by the respective standard certificates as recommended by the national agency in charge of vital statistics; provided, however, that the provisions of section 39-1005 , Idaho Code, shall be given effect on a certificate to which that section is applicable.
